Diagnosis requires laboratory evaluation—including measurement of serum cortisol levels and its circadian rhythm—and imaging studies to identify the source of excess cortisol. Therapeutic intervention is tailored to the specific etiology and can encompass pharmacological agents such as ketoconazole or metyrapone, surgical resection of adrenal or pituitary adenomas, and/or radiation therapy. Careful management of comorbidities such as hypertension and diabetes is also critical.
